Please remember that in the next step you will need to enter the client’s name, whether that is you or someone else. The phone number and email address entered must belong to the person who will be completing the forms in Step Two of our booking process. The forms we email you also include space to add additional phone numbers, extra email addresses, and any other information you feel we may need. (Note: These forms must be digitally signed and can’t be forwarded to another email or phone number for completion.)

If you are making multiple bookings for different clients on the same day, with same email and phone number, please follow up with an email to bookings@cccghm.com listing each client’s name and phone number. We will manually need to send the correct forms for each booking. Because the same email address and phone number must be used during the booking process, our system will not let you complete the forms more than once, as it recognises that your details are already in the system. We will update this manually on our end to ensure each client receives their own forms.

If the required information is not received within 5 days of the booking being made, the booking will be cancelled and a new booking will need to be submitted when you are ready
